1. Reservation Method

2. Precautions regarding the reservation
* Please reserve first and second dose AT THE SAME TIME
* You are able to reserve/cancel until the day before the vaccination (~23:59)
* Please make sure that your reservation does not overlap with the reservation at
the local government
* If you already got your first dose through the local government, you are not able
to get vaccinated at Waseda University
* Please use the local vaccination program if you are able to get the first does at
Waseda but not able to get the second dose.
3. Required Materials for Vaccination
1) Vaccination Ticket (接種券) issued by the local government
2) Personal Identification (for Current Students)
: Student ID / Driver's license / Health insurance card / Passport / My number card /
Residence card
3) Reservation Screen (either print-out or smartphone screen)
4) Prevaccination Screening Questionnaire (予診票)
5) Record of Vaccination for COVID-19 (新型コロナワクチン接種記録書)
